Put the date the work is due on the left, and your name and homeroom on the right. If you use lined paper, start just below the top line. Put the header on the paper before you start writing.
Below is an example of a good paper:

Some rules for good written answers in Dr. Turner's class:
1. Put the question in the answer. That means,
if the question is, "Name three types of
animals," your answer should begin:
"Three types of animals
are . . . " and go on from there. Don't
start sentences with "Because," "They," or "It."
2. Always answer in complete sentences, with an
upper case letter at the beginning and a period
at the end.
3. Type or write in your best handwriting.
Spell correctly. Use good grammar.
4. Write clearly. If you type your homework on
a typewriter or computer, it should look like
this page and have a proper header. If you write by hand,
use cursive and regular three-ring lined paper.
5. Be neat. Do not hand in paper which is wrinkled or has
torn edges or was ripped out of a spiral notebook.
If your dog chewed it, re-copy it.
6. If you have read this whole page, tell Dr. Turner
and she'll jump up and down with happiness. Guaranteed.
